What changes to Medicare Part D for 2027?

Three numbers change for Medicare Part D in 2027, and one program ends.

The annual out-of-pocket cap on covered prescription drugs rises to $2,400, from $2,100 in 2026. Once you hit that amount, you won’t pay anything more for the rest of the year.

The maximum Part D deductible rises to $700, from $615. Plans may charge less.

The national base beneficiary premium, which is used to calculate late enrollment penalties, rises to $41.33, from $38.99. Your own plan premium is set by the plan, not by this figure.

The federal premium stabilization program that held down stand-alone Part D premiums in 2025 and 2026 ends after 2026 and will not apply to 2027 plans. Average premiums are projected to stay close to flat, but individual plans can move more than the average.

The Medicare Prescription Payment Plan, which spreads your out-of-pocket costs into monthly payments, continues in 2027.

During open enrollment, October 15 to December 7, confirm that your medications are still on your plan’s formulary and compare its 2027 premium and deductible against alternatives.
